If it's a parallel dimension, then afkensshwajkebfgs mind rape. I'm opting for the parallel dimension idea. Murkoph can't be in Duane's head. That would make him a part of Duane, and that can't be possible. R-right? Okay now I'm picturing Murkoph as a catalyst for Duane's slow disintegration. Why mind?! Why you plague me with more questions I know Ashley won't answer?!
Finally. I was anticipating his arrival. And it does not disappoint. I’m exited to see how the readers who don’t know him will like him. But also if and how we, who only know bits and pieces (no pun intended), will change our minds regarding him.
@Terry: no, definitely not. That was just a random Aldish soldier. Murkoph is some uncontrollable zombie who does whatever he wants, and what he wants to do is murder stuff while waxing pseudo-philosophically about how he has found true freedom in becoming life's worst nightmare or something.
To be honest, I'd rather Sette would run away from that dude and not drag him back to the physical world.

Theory: Inside Duane's shadow is essentially like being inside a soul, in this case Duane's soul. Everything in there is some sort of reflection or symbol or exaggeration of how Duane sees himself and his own body. It's a pocket dimension, to which real people can go to. I have a feeling that Duane had an epic battle with this Murkoph and banished him there.

First off, I started liking you before I knew Unsounded existed--I favorited your "The East Wing" aaaages ago. Second... I am not officially worried about this new development. RUN SETTE RUN.

He's going to blow Sette's mind - he talks non-stop like Duane, but all crude and drawling like the people she's used to being around.
